Forest&Ray Payment Terms and Conditions

We at Forest&Ray guarantee that we provide the best quality and standard of care available in today's modern dentistry.

We offer long-term guarantee on all work carried out at our clinic.

Please ask for more information on guarantee details relating tospecific type of dental restorations at your initial consultation.

Please note, that the following payment options are acceptable ONLY at our clinic. A technical deposit of 50 percent of all final dental restorative work will be charged at time of preparation appointment, this will be noted on your treatment plan per visit.

Payment options:

• cash

• cheque (you will be asked to sign a declaration of obligation for all payments made by cheque, until final cheque transaction, upon finalization of payment the declaration of obligation will be cancelled, and filed with your documents.)

• bank transfer (please ask for our bank details for your transfer, and as above, you will be asked to sign a declaration of obligation until completion of transaction.)

Please note, that the above mentioned terms and conditions apply to all cases as company protocol, and you will be informed of all other relating detail as per case request.
